## Description

Healthcare Recruiters seeks one **full-time Mobile Physical Therapist.** Our client is headquartered in Golden Valley, MN but this position covers the **MANKATO** area **in MN. Must be willing to drive up to a 50 mile radius around this area.**


**$15K Retention/Sign-on Bonus paid out every 3 months during the first year of employment for full-time.

**Full benefits for full-time only. Choice to work weekends and or holidays for extra earnings - not mandatory!


**This is a traveling role and pay per visit salary structure with *mileage reimbursement* between client/in-home locations. Very *flexible schedule* with *field-based autonomy*. Supportive leadership and opportunities for growth.**

**Pay Scale: Potential annual compensation:** approx. $100,000–$210,000 annually (paid per visit).


Start of Care = $100.00 per Visit (Weekday) / $120.00 per Visit (Weekend)


Resumption of Care / Recertification of OASIS = $85.00 per Visit

Evaluations / OASIS Discharge = $80.00 per Visit

Functional Reassessment / Discharge = $50.00 per Visit

Treatment Visit / Supervision Visit = $50.00 per Visit

Meeting / Orientation Rate = $37.00 per Hour


**Primary Responsibilities**





- Evaluate client function and mobility through in-home assessments.

- Design and implement patient-specific rehabilitation strategies.

- Provide hands-on therapy, exercise instruction, and equipment recommendations.

- Collaborate with nurses, aides, and family caregivers.


**Requirements**





- Current **MN license** as a Physical Therapist.

- New Grads considered but prefer minimum one year experience.

- Must want to work in homecare.

- Valid driver’s license and reliable vehicle for travel.

- **Home health experience preferred**, but outpatient/inpatient backgrounds are welcome.

- Passion for **client-centered rehabilitation.**
