The NP/PA will provide comprehensive healthcare services in collaboration with our physician team. This provider will be responsible for assessing, diagnosing, and managing a wide range of conditions, primarily in an outpatient setting, with opportunities to participate in mobile diagnostics and specialty services depending on clinical interests and experience.

Key Responsibilities

  • Perform detailed patient histories, physical examinations, and comprehensive assessments.
  • Diagnose acute and chronic conditions using evidence-based clinical judgment.
  • Order, interpret, and follow up on diagnostic tests, including lab work, EKGs, and imaging.
  • Develop and implement individualized treatment plans, including prescribing medications.
  • Provide patient education and counseling regarding diagnosis, treatment plans, and preventive care.
  • Collaborate with physicians, specialists, technicians, and administrative teams for continuity of care.
  • Document all encounters thoroughly and accurately in the EMR system (e.g., eClinicalWorks or equivalent).
  • Monitor treatment response and adjust plans as needed.

Practice Scope & Environment

  • Setting: Primarily outpatient with options for hybrid/mobile diagnostic care
  • Patient Population: Adult and geriatric, with occasional adolescent cases
  • Specialty Services (varies by location): Internal Medicine, Primary Care, Cardiology, Geriatrics, Pain Management, Neurology, and more
  • Average Daily Patient Volume: [Insert number, e.g., 15–20 patients]
  • Schedule: [Insert days/hours – e.g., Monday–Friday, 9 AM – 5 PM, with occasional weekends]
  • Support: Dedicated administrative and medical assistant team; on-site diagnostics and lab integration


Required Qualifications

  • Master’s Degree or higher from an accredited Nurse Practitioner or Physician Assistant program
  • National certification (AANP, ANCC, or NCCPA)
  • Active and unrestricted New Jersey license (or ability to obtain)
  • Valid DEA registration (or eligibility to apply)
  • BLS and ACLS certification
  • Minimum 1–2 years of experience in primary care, internal medicine, urgent care, or relevant specialty (strongly preferred)
  • Proficiency with EMR documentation and workflow (eClinicalWorks experience is a plus)
  • Strong communication, organizational, and clinical decision-making skills

Preferred Qualifications

  • Experience with elderly or chronically ill populations
  • Experience with mobile care or multi-site care delivery
  • Familiarity with CMS quality metrics or value-based care
  • Bilingual skills (e.g., Spanish, Arabic, Hindi, etc.) are a plus
