Clinical Team Manager (RN) – CCM (Build & Scale, Remote)
Full-Time | $110K–$130K | Fully Remote (U.S.)
We’re hiring a Clinical Team Manager (RN) to build, structure, and scale a remote nursing team delivering chronic care management to complex patient populations.
This is a hands-on leadership role for someone who has built workflows, designed KPIs, and created operational structure—not just managed an existing team.
This is not a utilization review or task-based care role.
What You’ll Do
- Lead and scale a remote team of RNs/LPNs delivering longitudinal, panel-based care
- Design KPIs (daily/weekly/monthly) and drive team performance
- Build workflows, SOPs, QA processes, and training programs
- Track outcomes across patient engagement, documentation, and quality
- Identify gaps and implement operational improvements
- Support hiring, onboarding, and team scaling
What We’re Looking For
- Active RN license
- Experience managing remote clinical teams (non-negotiable)
- Experience leading teams in CCM, PCM, or population health
- Built or implemented KPIs, workflows, or team structure
- Strong in metrics-driven environments
- Experience with longitudinal/panel-based care
- Startup or program-build experience = strong plus
Not a Fit If
- You’ve only managed within fully established systems
- No hands-on experience building KPIs, SOPs, or workflows
- Primarily inpatient or non-longitudinal background
Compensation
- $110K–$130K base
- Fully remote
