Clinical Team Manager, RN (CCM/Population Health) – Remote
Full-Time | Fully Remote | $110k – $130k based on experience
We’re hiring a Clinical Team Manager, RN, to lead a remote nursing team delivering chronic care management services to patients with complex conditions.
This is a leadership role focused on team performance, quality oversight, and operational structure within a longitudinal care model.
What You’ll Do
- Manage and lead a team of remote nurses (RNs/LPNs)
- Oversee panel-based patient care with ongoing follow-up
- Monitor and drive performance metrics, quality, and productivity
- Ensure consistency across documentation and patient interactions
- Coach, mentor, and develop clinical staff
- Build and improve workflows, SOPs, and care processes
- Support hiring, onboarding, and team growth
Requirements
- Active Registered Nurse (RN) license
- Prior experience leading or managing nurses
- Background in CCM, PCM, care coordination, or population health
- Experience in structured, metrics-driven environments
- Strong understanding of longitudinal/panel-based care
- Remote leadership experience preferred
Not a Fit If:
- Primarily hospital or inpatient leadership background
- No prior responsibility for managing nurse performance
- Experience limited to utilization review or non-longitudinal care
Compensation
- $110,000 – $130,000 base salary
- Fully remote (U.S.-based)
