Licensed Practical Nurse (LPN/LVN) – Remote Neurology Care Coordination

Full-Time | Fully Remote | Compact License (NLC) Required
$55,000–$75,000 DOE

We’re hiring compassionate and clinically strong LPNs/LVNs to support patients with chronic neurological conditions including Alzheimer’s disease, Parkinson’s disease, multiple sclerosis, and cognitive impairment in a fully remote care environment.

LPNs/LVNs begin by supporting patient outreach, engagement, onboarding, and follow-up coordination before transitioning further into ongoing care support responsibilities as patient populations grow.

This role is ideal for nurses who enjoy patient communication, education, relationship-building, and structured remote workflows while continuing to grow clinically and professionally.

What You’ll Do

  • Conduct structured patient outreach and follow-up calls
  • Support patient onboarding and engagement
  • Reinforce care plans and patient education
  • Identify and escalate patient concerns appropriately
  • Coordinate scheduling, referrals, and follow-up needs
  • Document all patient interactions accurately in digital systems
  • Build relationships with patients and caregivers over time

What We’re Looking For

  • Active LPN/LVN license with Compact (NLC) privileges
  • Strong communication and patient engagement skills
  • Clinical experience working with chronic or medically complex patients
  • Comfort working in a structured, remote environment
  • Strong documentation and organizational skills
  • Ability to work independently and manage workflow expectations

Preferred Backgrounds

  • Hospital, clinic, rehabilitation, neurology, or chronic care experience
  • Experience with elderly or Medicare-aged populations
  • Experience in care coordination, patient outreach, or chronic disease management
  • Bilingual skills are a plus
