Remote RN Care Manager (CCM/Population Health)
Full-Time | Fully Remote | Compact License Preferred
We’re hiring experienced Registered Nurse (RN) to support patients with chronic neurological conditions (Alzheimer’s, Parkinson’s, MS) in a structured, fully remote care model.
This role is ONLY for RNs with prior care management experience (CCM, population health, or telephonic case management).If your background is primarily bedside, ER, OR, or utilization review, this will not be a fit.
What You’ll Do
- Manage a panel of patients over time and conduct ongoing follow-up, not one-time interactions.
- Conduct telephonic intake + follow-up calls
- Develop and manage care plans
- Perform ongoing check-ins and coordination
- Identify barriers and support patient engagement
What We’re Looking For
- Registered Nurse, RN
- 2+ years of telephonic or panel-based care management experience
- Experience managing chronic/complex patients
- Comfortable in a structured, metric-driven remote environment
Not a Fit If You:
- Are primarily bedside, ER, OR, or inpatient
- Work mainly in utilization review or audits
- Prefer episodic vs ongoing patient care
