Registered Nurse (RN) – Remote Chronic Neurology Support | Care Coordination

$90,000–$120,000 DOE
We’re hiring Registered Nurses (RNs) to support patients with chronic neurological conditions, including Alzheimer’s disease, Parkinson’s disease, multiple sclerosis, stroke-related conditions, and cognitive impairment, in a structured, fully remote care environment.
This role combines patient outreach, longitudinal care coordination, education, and ongoing engagement. RNs work directly with patients and caregivers and partner closely with an interdisciplinary team in a technology-enabled, metrics-driven model focused on improving long-term outcomes.

What You’ll Do
- Conduct telephonic patient outreach and ongoing follow-up
- Support patients longitudinally through education, care coordination, and engagement
- Reinforce care plans and identify changes from baseline
- Escalate clinical concerns appropriately
- Collaborate with interdisciplinary teams to support patient outcomes
- Document all interactions accurately within structured workflows
- Build trusted relationships with patients and caregivers over time
What We’re Looking For
- Active RN license (Compact/NLC preferred)
- Strong clinical nursing background
- Experience working with chronic or medically complex patients
- Excellent communication and patient education skills
- Comfortable in a structured, technology-driven remote environment
- Strong documentation and organizational skills
- Ability to work independently while managing workflow expectations
Preferred Backgrounds
- ICU, Neuro, Stepdown, Telemetry, PCU, Med-Surg, or Care Coordination experience
- Experience with elderly or Medicare-aged populations
- Exposure to neurological conditions or chronic disease management
- Experience in educating patients and caregivers
