Remote LPN/LVN – Chronic Neurology Care Coordination

Full-Time | Fully Remote | Compact License (NLC) Required

$26-$40/hour depending on experience and clinical background

We’re hiring experienced LPNs/LVNs to support patients with chronic neurological conditions such as Alzheimer’s, Parkinson’s, and multiple sclerosis.

This is a fully remote role for nurses who can build relationships over time, assess changes from baseline, and confidently guide patients and caregivers through ongoing care. You’ll combine structured outreach, monthly check-ins, and care coordination to support patients longitudinally.

This is not a home health or visit-based role.

What You’ll Do

  • Manage a longitudinal patient panel with ongoing monthly check-ins
  • Support patients with neurological conditions through education and care guidance
  • Assess for changes from baseline and escalate concerns appropriately
  • Communicate with caregivers to improve care and patient outcomes
  • Coordinate referrals, appointments, and medication-related needs
  • Recommend disease-specific and local resources (lifestyle, support, community-based care)
  • Document all interactions in a structured, metrics-driven workflow
  • Partner with internal teams by providing feedback to improve care delivery

What We’re Looking For

  • Active LPN/LVN license with Compact (NLC) privileges
  • Clinical experience working with patients with neurological or chronic conditions
  • Ability to clearly explain conditions, progression, and care strategies to patients and families
  • Experience with ongoing patient follow-up, care coordination, or case management
  • Strong documentation skills and comfort in a structured, remote environment
  • Ability to work independently while managing a consistent workflow

Strongly Preferred:

  • Experience in clinic, hospital, or care management settings
  • Exposure to CCM and/or PCM programs
  • Experience managing a patient panel over time (longitudinal care)
